Periosteal osteosarcoma is a rare form of osteosarcoma that mostly develops at the surface of long bones exhibiting distinct radiographic and histologic features and biologic behavior different from that of conventional osteosarcoma. Wide resection is the recommended treatment, while marginal resection leads to a high risk of recurrence. Periosteal osteosarcoma is an intermediate-grade osteosarcoma and it has better prognosis than other conventional osteosarcomas. This paper describes the major clinical, radiographic and histologic features of periosteal osteosarcoma and illustrates these with the case of a 34-year-old female presenting with recurrent periosteal osteosarcoma in the proximal tibia.
