脊柱内窥镜技术治疗腰椎间盘突出症的应用进展(英文)

Lumbar disc herniation (LDH) is a common and frequently-occurring disease. The traditional treatment method of LDH is Lamina decompression discectomy, but this kind of operation has some unavoidable complications such as nerve root adhesion, spinal instability and so on. During the past 10 years, spine endoscope technology is used more and more widely because of its feature of being minimally invasive. The spine endoscopic technologies are used in the treatment of LDH currently including microendoscopic discectomy (MED), percutaneous transforaminal endoscopic discectomy (PTED) and full-endoscopic interlaminar disectomy (FLD). The purpose of this paper is to review the progress of these minimally invasive technologies. Compared with traditional open surgery, they all have advantages including small trauma, less bleeding, quick postoperative recover and less complications. They represent the direction of the spine minimally invasive surgery. Especially, PTED and FLD keep the stability of the column frame structure of the spine effectively. We should strictly master their principles, indications and contraindications when we use these technologies for the treatment of LDH. We should take an individual treatment plan rather than expanding the indications blindly. For the same technology used in different patients, the puncture point, angle and position should be personalized, emphasizing the targeted biopsy. And then, we can get better clinical effect.
