Femoral intertrochanteric fractures occur mainly to elderly patients with osteoporosis. With the socio-economic development and speedy population aging, the incidence rate of femoral intertrochanteric fractures increases year by year. In recent years, with the progress of internal fixation technology and the development of equipment, the treatment outcomes of intertrochanteric fractures have been greatly improved. However, the rate of postoperative complications which are caused by the malformation of hip varus and long-term bedridden is still higher. Therefore, it is essential to treat intertrochanteric fractures. There is conservative treatment, surgical treatment and semi-invasive threadneedle fixation with external fixator ranging between surgical and nonsurgical. The treatment method should be chosen according to the age, health status, type of fracture, degree of osteoporosis and so on. In principle, patients who have greater tolerance of surgical procedure should be treated surgically as soon as possible. For the stable femoral intertrochanteric fractures, dynamic hip screw (DHS) and other screw-plate internal fixation systems can be chosen. For the unstable ones, gamma nail, proximal femoral nail (PFN) and other intramedullary fixed systems can be chosen. For anti-intertrochanteric fractures, dynamic condylar screw (DCS) can be used. Elderly patients with comminuted intertrochanteric fractures of the femur can be treated with artificial joint replacement. With the deepening understanding of femoral intertrochanteric fractures and the popularization of new equipment and methods, the treatment for fractures will be more and more successful, and the trauma and complications will be fewer and fewer.
